I authorize my child to participate in Kids Music N’ Motion. I agree to pay the fee listed in "Class Descriptions" and understand that there are no make-ups if my child is ill or cannot make the class. I also understand that there will be no refunds if my child is ill or cannot make the class. I am also aware that classes are tentative based on enrollment.
